We offer a range of services including Harm Reduction and Primary Health Services as well as a range of treatment types (including residential treatment centres, community-based support programs, counselling, and withdrawal management services).

About the role

We are seeking a values-driven Support Worker who will provide support to people, who present for assistance, in a community based and / or residential based setting. Under the direction of senior staff, the role provides support to the strategies developed within the persons/families' case management plan.

This support incorporates person centred, resilience, trauma informed and strengths-based practices whilst working effectively with other members of the team to provide a comprehensive service that reflects the vision, mission, and values of The Salvation Army.

You will successfully

  • Establishing relationships with individuals that access the service that encourage self-determination and participation
  • Support the strategies developed within the individuals case management plan in accordance with TSA case management models and standards of practice
  • Ability to complete thorough assessments, inclusive of needs, risks, and vulnerabilities.
  • Support in the ongoing assessment of the individuals' presenting issues, vulnerabilities and needs from initial intake to exit
  • Support the individual case planning in collaboration with the individual including a range of interventions that address the needs or goals of the individual, as well as barriers to achieving those.

You will have

  • Tertiary qualification in Community Services, Social Work, or related field, with a minimum of a diploma level.
  • Desirable to have relevant experience in a social service environment [e.g. homelessness]
  • QLD Working with Children Check is required
  • Driver's Licence
  • First Aid Certificate and CPR
  • Be available for night/weekend and sleep over shifts.

This is a part-time (26hrs per week), permanent position located in Ashmore City, QLD. Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services CRISIS Level 1.
